Annie Knowlton

Annie is our 2019 scholarship winner. She was delivered by our very own Dr. Ken Ostermann right here in Beaver Dam. In Fall 2019, Annie will pursue her passion for the arts at Trinity International University (in Deerfield, Illinois) taking courses in Graphic Design and Marketing. She plans to use her graphic design skills to help various non-profits and other organizations close to her heart.

Simar Garcha

Simar is one of two achievement award winners. This fall, she will attend the University of Southern California to study Global and Public Health. Simar’s dream is to work for Doctors Without Borders, where she can make a difference in health policies worldwide.

Madelynn Johnson

Madelynn is another achievement award winner. She was delivered by our very own Dr. Ken Ostermann, right here in Beaver Dam. Madelynn is passionate about making the world a better place. This fall, she will attend Winona State University and plans to study Therapeutic Recreation.

 

High School Seniors! You could win a $1,000/year college scholarship (for up to four years) if you attend school and live in the Beaver Dam area.

Fill out the form below and submit your short essay to be entered to win! New this year—we are accepting a résumé or list highlighting extracurricular involvement and employment. Enter now—this contest starts February 15, 2019, and ends April 15, 2019. The winner will be announced in early May!

Please answer the following question with a short essay. There is no word limit, and answers will be evaluated based on the quality of your response and how well it relates to your intended field of study and academic goals.

What are your plans for education after high school and how do you hope to use your education to make the world a better place?

Requirements to enter
Applicants must:

  • Be a current high school senior at a public or private high school within 15 miles of Beaver Dam
  • Have primary residence within 15 miles of Beaver Dam
  • Provide a current transcript from your high school**
  • Submit a short essay in response to the question above
  • *Agree to terms and conditions of scholarship - Entrants under 18 years of age must have a parent or guardian print this form and mail it to Beaver Dam Women's Health
  • The winner must provide a transcript or proof of good academic standing from their college or technical school each fall in order to receive scholarship funds in subsequent years
  • Agree to an interview with the Beaver Dam Women's Health team (winner and/or finalists only)
  • Winner and/or finalists will be expected to be available on May 2nd for photographs (if unavailable, please let us know in your application)
