We provide comprehensive women’s health services by appointment only. Same day appointments are often available. To schedule an appointment, please email, call (920)885-6090 or visit our Patient Portal to take advantage of our secure online services, including appointment requests.
On the day of your appointment, please arrive 10 minutes early so that we can ensure you receive the prompt and personalized attention you deserve. If you are visiting our clinic for the first time, please download and complete our Patient Registration Form and our Health History Form.
Cancellation Policy
In order to provide timely and quality care to each patient, we request 24 hours notice to cancel or reschedule your appointment.
BDWH will respond as promptly as scheduling allows to set-up your appointment. If you believe that you have a medical issue that is urgent, you should call BDWH to schedule an appointment. If you believe that you may require immediate medical attention, you should call 911.
Receptionists are available to help you schedule an appointment Monday – Friday 8:00am – 5:00pm. Call 920-885-6090. Please have insurance and contact information available. If this is your first visit with us, please download and complete our Patient Registration and Patent History forms and bring them to your appointment.
